In Buckinghamshire, people with a learning disability have been using Health Passports since 2005.

The Health Passport was made for and with people with a learning disability.

On these pages you will be able to find out more about the Health passport and how you can get one.

The Health Passport is available free for anyone with a learning disability in Buckinghamshire.

 

 For You

The Health Passport was made to help you look after your health and can support the people around you to have the best information to help you look after yourself.

You may want another person you know and trust to work with you when you fill in your Health Passport,

this person may be a:

-Family member

-Key worker

-Community Nurse

-Friend

You should know and trust the person who supports you with your Health Passport.

A Health Passport can make it easier for you to use health services, perhaps when you go to see your doctor or dentist or when you have to go to the hospital.

Make sure that you take your Health Passport with you when you go to your health appointments or when you have to stay in hospital.

The Health Passport belongs to you. Remember to keep it in a safe place.

The Health Passport was made to improve the health with all people with a learning disability.

A Health Passport can empower people:

-by providing information in a format that is easier to understand

-by enabling the people that support them to have better information about the health needs of the individual and the best way to help them

Most people with a learning disability need some sort of support. Some people require a lot of support throughout their lives. Other people will need a little bit of help with their everyday living. Some people are able to communicate their needs in all situations. Other people may find this harder to do and therefore need more support.

The Health Passport was written with a learning disability in mind.

Some people may use the Health Passport quite independently.

For other people a Health Passport may be used to help with the sharing of information between those who support the individual with their healthcare professionals.

Make sure that the person you are supporting takes their Health passport with them when they go to their health appointments or when they have to stay in hospital.
